MTA for Receiving Samples

When receiving samples from NCRAD the “Master Agreement for Transfer of Materials from NCRAD” and related Appendices must be completed prior to the receipt of any samples. Please note that a separate Master Agreement must be executed by your Institution for each Investigator requesting NCRAD Research Materials. However, if you think your Investigator may already have a Master Material Transfer Agreement (MTA) in place with NCRAD, please email us at alzstudy@iu.edu to check before initiating the process.

  1. Please forward this information along with a copy of the Master MTA to the office at your Institution responsible for review and official authorization of Material Transfer Agreements. A template email for you to send to your MTA office is available for download in the sidebar to the right.

    Please Note: The terms of this Master Agreement and Appendices are not negotiable. We have hundreds of investigators that we work with and require all Master Agreements to be completed under the same terms and conditions.

  2. When your MTA office has signed the Master MTA, please forward the partially executed agreement to alzstudy@iu.edu.

  3. NCRAD will complete the signature process and return a PDF copy of the fully executed Master MTA to the responsible parties.

An Appendix C and G must be completed for each request. These documents are addendums to the Master Agreement signed between Indiana University and your Institution.

  1. The Effective Date of the Master Agreement is the date of the last signature on the Master Agreement. Please insert the Effective Date within the first paragraph located in the body of the document where indicated.

  2. Please complete all of the questions in section 5 of the Appendix C. A list of samples being requested should be included in response to 5b. This may be attached as a separate page when necessary. \

    Please Note: The Principal Investigator is the one who will need to sign the Appendix C. The Trustees of Indiana University do not require this document to be signed by your Institution. However, you should check with your Institution’s MTA office to determine whether your institution requires review and institutional signature on this Appendix C. The Trustees of Indiana University will not be reviewing or signing the Appendix C again as there are no changes in the terms between this agreement and the Master Agreement. Only the NCRAD Principal Investigator will be signing this document from our end.

  3. Please check the box(es) on the Appendix G related to the sample set(s) you are requesting samples from. This is the acknowledgement language that will be required when publishing on the samples and data obtained from this NCRAD request.

  4. When your Principal Investigator has signed the Appendix C and completed the Appendix G, please forward the partially executed agreements to alzstudy@iu.edu.

  5. NCRAD will complete the signature process and return a PDF copy of the fully executed Appendices to the responsible parties.

The Appendix F is used when the Recipient Institution transfers Derived Material to NCRAD. For example, when a Recipient requests Peripheral Blood Mononuclear Cells (PBMCs) to generate Induced Pluripotent Stem Cells (IPSCs), at least two aliquots of the IPSCs are required to be returned to NCRAD for sharing. The Appendix F covers the terms of this transfer.

  1. The Effective Date of the Appendix F is the date of the last signature of the Appendix F document. Please note that the Effective Date will be entered by the NCRAD staff upon receipt of the partially executed Appendix F.

  2. A list of the Derived Materials should be included in response to section 3. This may be attached as a separate page when necessary.

  3. Please forward this information along with a copy of the Master MTA to the office at your Institution responsible for review and official authorization of Material Transfer Agreements.

An Appendix I must be completed for each request of iPSC samples. This document is an addendum to the Master Agreement signed between Indiana University and your Institution.

  1. The Effective Date of the Master Agreement is the date of the last signature on the Master Agreement. Please insert the Effective Date within the first paragraph located in the body of the document where indicated.

  2. The Principal Investigator is the one who will need to sign the Appendix I. The Trustees of Indiana University do not require this document to be signed by your Institution. However, you should check with your Institution’s MTA office to determine whether your institution requires review and institutional signature on this Appendix I. The Trustees of Indiana University will not be reviewing or signing the Appendix I again as there are no changes in the terms between this agreement and the Master Agreement. Only the NCRAD Principal Investigator will be signing this document from our end.

  3. When your MTA has signed the Appendix I, please forward the partially executed agreement to alzstudy@iu.edu.

  4. NCRAD will complete the signature process and return a PDF copy of the fully executed Appendix I to the responsible parties.
