Study Resources
Access the ALLFTD2 Kit Request Module. This link will direct you to a REDCap database where study coordinators and staff may request kits, individual supplies, and/or labels.
Please allow a total of three weeks for kit requests to be compiled and delivered to your site.
Please use the online sample form to collect information on specimen demographics, collection, and processing. After submitting the form online, download and print the PDF to include a hard copy with each shipment. In addition, notify NCRAD in advance of shipment by submitting the NCRAD Specimen Shipment Notification Form online. An email will automatically be sent to alzstudy@iu.edu once submitted.
If you are unable to access the online form at the time of collection, you may print and complete the previous form as an interim option, but please ensure the online form is submitted as soon as computer access is available.
The downloadable manual for biofluids was created specifically for the ALLFTD2 Study. Please feel free to explore the manual through the linked ‘Table of Contents.’ Questions concerning any part of the manual may be directed at NCRAD alzstudy@iu.edu, 1-800-526-2839 or directly at 317-278-8413 for further clarification.
